Bug Description
Weekly limit reset correctly today, Oct 8 2026, 5:00am (America/Bogota), but without sending a single prompt today: weekly usage already shows 16%, and session usage shows 100% (resets 3pm). This has been happening consistently since last week — I got fully blocked by the weekly limit last week under the same pattern, with minimal actual usage.

Verified: no other Claude Code sessions/terminals running on this machine, no automations (n8n/MCP/webhooks) configured to call Claude Code. /context for the current session shows only 6% context used (53.8k/967k tokens), confirming no local activity that would justify this consumption.

Environment: Claude Code v2.1.226, model Sonnet 5 (claude-sonnet-5), timezone America/Bogota.

Impact: blocked from working for the entire week last week, and again today, despite negligible real usage.
