/ultrareview run crashed without producing findings on
May 4, 2026 (Madrid time). Run was consumed despite zero output.

review went to background, returned later as crashed with no findings.
Free run was consumed.

This appears consistent with the documented infra-side failure pattern
reported in #54871, #54812, #53455, #54516, #49905.
Requesting refund of consumed free run.
