ultrareview crashed before producing findings — free credit consumed

Resolved 💬 19 comments Opened Apr 24, 2026 by JoanCremades Closed Jul 16, 2026

Ran /ultrareview on a branch and the run crashed before producing any findings, but it still consumed 1 of my 3 free ultrareviews.

Session: session_01HXqHPAdJurM7MVo5gFFGXU

Error returned to the CLI:

{"error":"Review crashed before producing findings. See session logs for details."}

Scope of the review:

  • 302 files changed
  • 9858 insertions(+)
  • 11331 deletions(-)

Request: please refund the free ultrareview credit, since the run produced no output.

Environment:

  • Claude Code CLI
  • Platform: darwin (macOS 25.4.0)
  • Model: Opus 4.7 (1M context)

Update: reproducible — second crash on the same branch.

Relaunched /ultrareview on the same branch and it crashed again with the identical error. Same symptom, same scope, different session. This looks like a runner bug triggered by the diff size (302 files / ~21k lines changed), not a transient failure.

Second session: session_016RwQU4xS9AovXu2pFCsJ6V

Error (identical to first run):

{"error":"Review crashed before producing findings. See session logs for details."}

2 of my 3 free ultrareviews have now been consumed on this branch with zero findings produced. Requesting both credits be refunded, and flagging that large diffs appear to reliably crash the runner.

Same issue here. Running /ultrareview on a branch with 67 files changed, 6623 insertions, 858 deletions consumed 1 of
my 3 free credits but crashed before producing any findings. Running a second now.

Error returned:
{"error":"Review crashed before producing findings. See session logs for details."}

Session: session_01KYAft8b7K8JZhVB91W35sy
Platform: Windows 10
Model: Claude Sonnet 4.6

Requesting a credit refund for the failed review. Happy to share any additional logs if helpful.

Update: ran it again and got the same crash. Appears reproducible on my end.

Session: session_011tCrZ6YBCJBnJnYkPAcHuY

This consumed a second free credit with no output. Now down to 1 of 3 remaining. Holding off on the third until
there's a fix or response.

Same crash on macOS. Branch experiment/web-socketmain, 146 files / +4,777 / -1.

Session:
session_015M5YyNChCn2mKBaLaR59Yq
Model: claude-opus-4-7 (1M context). Free credit consumed.

+1 on refund request.

+1, same crash pattern on macOS / Claude Opus 4.7.

4 consecutive crashes on the same project, all at the "Find" step with
the identical generic error 'Review crashed before producing findings'.
Tested across two different PRs in the same repo.

So the issue does not appear to be tied to PR size. Same error on a
4K-line PR as on a 17K-line one.

Sessions affected (paid runs):

  • session_01NcC6zGu6NQNphVfsfSqHx5
  • session_017fCSBXGJQmtYVjQd9V6PqH

Requesting refund for the 2 billed runs that produced no output.
Happy to share more session logs.

Likewise, session_0127jCPu2QYTM3LrZYNgBDdp, Opus 4.7., MacOS I'd appreciate it if we could have credit refunds for these

Same here, 1 of 3 free runs was consumed. #55259

+1 — same issue, all 3 free credits consumed with zero findings produced.

Sessions (3 crashes in a row, same branch):

  • session_01J852qegxTdn1u13qecGeDL
  • session_016SBcXSotXHSofYZkjo8XGR
  • 3rd session ID not surfaced in CLI output

Error returned each time:

{"error":"Review crashed before producing findings. See session logs for details."}

Scope of review: large branch — 302 files / ~21k lines, similar profile to other reports here. Reproducible.

Environment:

  • Claude Code CLI, Opus 4.7 (1M context)
  • Platform: darwin (macOS 25.0.0)
  • Target: PR #1 on aminechouikham-beep/servicelink-marketplace-47-

Requesting refund of all 3 consumed free credits since no findings were ever produced.

And the quota was also spent, although ultrareview actually did nothing for me.

Happened twice to me, filed the bug and hoping to get the free runs back

Same issue, requesting two refunds:

https://claude.ai/code/session_01QkpKutVcmW22qaJEZ6dLRx

Going through support is like talking to a wall and results in "I apologize for the disruption in service you experienced with those ultrareviews. We take service reliability very seriously, but we are unable to issue compensation for degraded service. We are consistently working on improvements to our infrastructure to better handle high traffic volume in order to prevent similar issues from happening in the future. Thank you for your patience."

Same issue here as well. Requesting Credit Refund.
Both of these failed, got 0 result.

Both received: ● API Error: Server is temporarily limiting requests (not your usage limit) · Rate limited
