Update: reproducible — second crash on the same branch.
Relaunched
/ultrareviewon the same branch and it crashed again with the identical error. Same symptom, same scope, different session. This looks like a runner bug triggered by the diff size (302 files / ~21k lines changed), not a transient failure.Second session:
session_016RwQU4xS9AovXu2pFCsJ6VError (identical to first run):
2 of my 3 free ultrareviews have now been consumed on this branch with zero findings produced. Requesting both credits be refunded, and flagging that large diffs appear to reliably crash the runner.

Same issue here —
/ultrareviewcrashed with the identical error and stillconsumed one of my 3 free runs.

Same issue here. Running /ultrareview on a branch with 67 files changed, 6623 insertions, 858 deletions consumed 1 of
my 3 free credits but crashed before producing any findings. Running a second now.

Requesting a credit refund for the failed review. Happy to share any additional logs if helpful.
Update: ran it again and got the same crash. Appears reproducible on my end.

This consumed a second free credit with no output. Now down to 1 of 3 remaining. Holding off on the third until
there's a fix or response.
Same crash on macOS. Branch

+1 on refund request.
+1, same crash pattern on macOS / Claude Opus 4.7.
4 consecutive crashes on the same project, all at the "Find" step with
the identical generic error 'Review crashed before producing findings'.
Tested across two different PRs in the same repo.
So the issue does not appear to be tied to PR size. Same error on a
4K-line PR as on a 17K-line one.
Sessions affected (paid runs):
Requesting refund for the 2 billed runs that produced no output.
Happy to share more session logs.
Likewise,
session_0127jCPu2QYTM3LrZYNgBDdp, Opus 4.7., MacOS I'd appreciate it if we could have credit refunds for thesesame issue here. And it's been deducting from teh free credits. will they get re-instated?
Same here, 1 of 3 free runs was consumed. #55259
+1 — same issue, all 3 free credits consumed with zero findings produced.

And the quota was also spent, although ultrareview actually did nothing for me.
Happened twice to me, filed the bug and hoping to get the free runs back

Going through support is like talking to a wall and results in "I apologize for the disruption in service you experienced with those ultrareviews. We take service reliability very seriously, but we are unable to issue compensation for degraded service. We are consistently working on improvements to our infrastructure to better handle high traffic volume in order to prevent similar issues from happening in the future. Thank you for your patience."
Same issue here as well. Requesting Credit Refund.
Both of these failed, got 0 result.
