ultrareview crashed but consumed free credit (no findings produced)

What happened

Ran /ultrareview 2 on yann909/Azura-pro#2. The command reported:

Free ultrareview 1 of 3. Ultrareview launched for yann909/Azura-pro#2 (~5-10 min, runs in the cloud).

~5 minutes later the task notification returned:

{"error":"Review crashed before producing findings. See session logs for details."}

Problem

• The review crashed before producing any findings
• The free credit (1 of 3) was consumed anyway
• The session page (https://claude.ai/code/session_011eLvyAkGy6Pr56KUQR4RGA) is not accessible for debugging from the user's side

Expected

• A crashed review with zero findings should not consume the free credit
• Or, at minimum, make the crash logs visible to the user so they can understand what went wrong
