/ultrareview free credit consumed despite remote session timeout (no output produced)

Free /ultrareview credit was consumed for a session that timed out after 30 minutes without producing any output. The retry was forced to use my last remaining credit, which is quite frustrating — I've effectively lost a review I never got to use.

Failed run:

Expected: A failed run that produces no output should not consume a free credit (or should be auto-refunded).

Actual: Quota dropped from "2 of 3" → "1 of 3" after the failure, then the second retry consumed the final credit ("3 of 3"), leaving zero remaining despite only one successful review.

Asks:

  1. Refund the credit lost to the timeout.
  2. Either raise the cloud-session timeout for large diffs (this PR has 60 commits ahead of main) or fail-fast with a clearer error before consuming the quota.
