[BUG] /ultrareview crashed before producing findings — free run consumed, expired same day

Description

\/ultrareview\ crashed before producing any findings, but consumed one of my 3 free runs. The free runs expired on May 5, 2026, so this is a time-sensitive refund request.

Steps to reproduce

1. Private GitHub repo (tippentopp/dj-automation-v13)
2. Ran /ultrareview 2 (PR #2)
3. Session started successfully: https://claude.ai/code/session_015rdTvxirR1jrhrGjqkN8iE
4. ~5-10 min later: task notification returned {"error":"Review crashed before producing findings. See session logs for details."}

Expected behavior

Review findings are delivered, or the free run is not consumed on infrastructure-side crash.

Actual behavior

No findings delivered. Free run quota decremented. Run expired same day (May 5, 2026).
