[BUG] ultrareview crashed and consumed a free use without producing findings

What's Wrong?

/ultrareview crashed and returned "Review crashed before producing findings. See session logs for details." This consumed one of my 3 free ultrareview uses without producing any results. The diff scope was ~210 files / ~65k insertions.

What Should Happen?

The ultrareview should either complete successfully or not count against the free usage quota if it crashes on the infrastructure side.
