[BUG] Artifact delete approval prompt never shown in Projects threads (iOS); thread blocks forever

What's Wrong?

Surface: Claude Projects (claude.ai), thread session in a cloud container, viewed in the Claude iOS app.
Tool: Artifact, action "delete".

When a thread session calls the Artifact tool to delete an artifact it published, the session parks on its confirmation ("permanently delete the artifact ... (irreversible)"). The project's coordinator session receives a requires-action notice for that thread, but no Allow/Deny control appears anywhere in the thread. The thread stays blocked until it is told to give up. Replying "yes" in the thread does nothing.

Other permission prompts in the same threads (Bash commands, including ones raised by a PreToolUse hook returning ask) do appear and can be approved from iOS. Only the Artifact delete confirmation is missing. It has happened on several days this past week, across different threads.

What Should Happen?

An Allow/Deny prompt in the thread, like other permission prompts; or, if deletes can't be approved from Projects, a clear message saying so and where to delete instead.

Steps to Reproduce

1. In a Claude Project, have a thread publish an artifact with the Artifact tool.
2. Ask that thread to delete the artifact.
3. Open the thread in the iOS app.
4. No prompt appears; the session blocks indefinitely, and the artifact can't be deleted from the phone (the iOS artifact viewer has no delete action either).

• #97456: same bug in Projects on Windows desktop and web. Auto-closed by the bot as "not related to Claude Code" with no maintainer review, so it is still unfixed.
