[Bug] Claude stops mid-task without completing stated actions

Bug Description
Opus5 just stops mid-task, often saying things like "next I will Y..." or "now i will Z" but then just stops instead of doing them, despite giving the clear task ahead of this. Not an api or network error, it just stops. Especially annoying after a multi-hour turn is given and opus just stops a few minutes in so you come back to see its just stopped itself
