What happened

/ultrareview 4 launched successfully and reported:

Free ultrareview 1 of 3.
Ultrareview launched for Spektra21/spektra-os#4 (~5-10 min, runs in the cloud).

A few minutes later the task failed with:

Cloud review failed: orchestrator reported an error: Review failed: all review agents terminated before completing.

No review output was produced at all. The free run still appears to have been consumed.

Expected

A run that crashes in the orchestrator and returns zero findings should not count against the free allowance.

Note

Getting to a working launch also took a long time: the failure message was
"GitHub repository access check failed - re-authorize GitHub in settings",
which does not say which settings. GitHub's own app page, the CLI (claude auth
has no GitHub subcommand) and /config were all dead ends; the actual fix was
connecting GitHub at claude.ai/code. A message naming that location would save
a lot of time.
