[BUG] iPhone app shows duplicate session entries after renaming a session

What's Wrong?

On the iPhone Claude Code app, in the Code tab, I have a session from today that spans two calendar days (started ~6/30, continued into 7/5). I renamed the session today (7/6) to update its label/date range from ending in "7/5" to "7/6" to reflect that it's still active.

After the rename, the session list now shows two entries for what should be a single session — one of them is stale/phantom but still displays a "Connected" status, even though that specific entry does not correspond to an actual running session.

What Should Happen?

Only one should appear on the phone, the way it does on the PC app.

Steps to Reproduce

  1. Have an active session that was renamed while in progress (label/date range changed mid-session).
  2. Open the Code tab on the iPhone app.
  3. Observe two entries where there should be one — one showing "Connected."

Troubleshooting already tried (did not resolve it):

  • Force-quit and reopened the app 3 times — duplicate persists.

Expected behavior: Renaming a session should update the existing entry in place, not leave behind a duplicate/stale row.

Actual behavior: Two entries appear for the same underlying session; one is a phantom that doesn't reflect the actual session state.
