[Bug] Ultra Code Review crashes on Anthropic API rate limit without refunding credits

Bug Description
Ultrareview (/code-review ultra) consumed a free credit but crashed before
producing any findings.
Result returned: {"error":"Review crashed before producing findings. See
session logs for details."}
Underlying error: "Server is temporarily limiting requests (not your usage
limit) · Rate limited"
