[BUG] 2 of 3 ultra review credits burned up because of rate limit errors in CC CLI. Now none are left when I really need them.

What's Wrong?
I needed to use an ultra review at a critical phase in my project. Kicked it off - it instantly failed, but showed 2/3 when I'd only actually used one a few weeks ago. When I tried it the next time around after an hour, same API rate limits error, but now showing 3/3 ultra review credits used.
What Should Happen?
Do not use up ultra review credits until the review is done - the bookkeeping should account for env-related errors or non-completion, and not charge an ultra review free credit in these cases.

Steps to Reproduce
Just run /ultrareview with a commit hash (see screenshot below). I cannot reproduce now by definition, since I'm out of the 3 free ultra reviews.
