[BUG] /ultrareview burned free quota on server-side rate-limit failure

What's Wrong?

Ran /ultrareview with my last free run (3 of 3). Got back empty findings [].
When I tried to re-run, server returned:

API Error: Server is temporarily limiting requests (not your usage limit) · Rate limited

Then:

Extra Usage credits exhausted. Top up to continue.

What Should Happen?

Failed runs (server rate limit, timeouts, etc.) shouldn't consume free quota.
