[BUG] Ultrareview consumed free quota despite returning empty findings

What's Wrong?

## Summary
Ultrareview run finished with empty findings array [] on a large diff (1053
files, 108,764 insertions), but the run was still counted against my free
monthly quota (1 of 3 used).

502 after 3 attempts. After /login, second invocation launched successfully
but returned empty findings.

## Expected

• A failed or empty-result review should not deduct from the free quota.
• Either: (a) refund the quota slot, or (b) re-run the review with proper

findings.
