[Bug] Claude Code Ultra review crashes with rate-limit error and consumes quota

Bug Description
I attempted to use a claude code review ultra and it immediately crashed. session_01GKfBKF3wEkL8rVoCA64f2S. Cloud review crashed before producing findings, then returned a rate-limit error — but it consumed 1 of 3 free ultrareviews.
