Accept Edits mode still prompts for edit permissions

Bug description

When "Accept Edits" mode is enabled (acceptEdits permission mode), the user is still being prompted to authorize edit operations. This is intermittent — the user reports it usually works fine but experienced repeated permission prompts in a session despite having the mode active.

Steps to reproduce

1. Enable "Accept Edits" mode in Claude Code
2. Start a session that involves multiple file edits (Edit, Write tools)
3. Observe that permission prompts still appear for edit operations

Expected behavior

With "Accept Edits" mode enabled, all Edit and Write tool calls should be automatically approved without prompting the user.

Actual behavior

The user is repeatedly prompted to approve edit operations despite having "Accept Edits" mode on. The user confirmed this usually works and is a regression or intermittent issue.
