[BUG] Closed conversation disappears from Past Conversations despite .jsonl file existing on disk

What's Wrong?

After completing a conversation in the VS Code extension and closing the editor
tab, the conversation no longer appears in the "Past Conversations" dropdown

What Should Happen?

Conversation should appear in Past Conversations list

Steps to Reproduce

1. Open Claude Code in VS Code
2. Have a multi-message conversation (this one involved Webflow form spam settings)
3. Close the editor tab
4. Open "Past Conversations" dropdown — the conversation is missing
5. Confirm the .jsonl file still exists on disk
