What happened (9 Oct 2026, ~12:00 ET)

The model called the GitHub connector's create_pull_request (my private skills repo, head systems-precision-class-not-instance → main).
The tool result told the model: "An interactive form has been shown to the user... Wait silently for the user to review and click Submit."
No form, card or button appeared anywhere in the chat. The user's screenshot of the full conversation shows only "Used GitHub" and the model's text.
The model, trusting the tool result, told the user a Submit button was waiting on them. It wasn't. The user was left with a blocked task and nothing to click.

Expected
Either the form renders, or the tool returns an error the model can act on. The tool result should never assert UI the user cannot see.
