Claude Docs: @Claude comment from the iOS app never reaches the Claude Code Desktop session that owns the doc ("Comment added, but it didn't reach Claude")

Summary

A Claude Doc created by a local Claude Code session in Claude Desktop (Code tab) can only receive @Claude comments sent from Claude Desktop on the same Mac. The same comment in the same thread, sent from the Claude iOS app, is saved to the doc but never reaches the owning session. The iOS app shows: "Comment added, but it didn't reach Claude. Try again from a reply, or ask Claude directly in your session." Retrying from a reply on iOS doesn't help either. Because of this, commenting on Code-created docs from a phone doesn't work, even though it is the normal way to use Claude Docs.

Environment

• Claude Desktop 2.16120.0 (macOS 26.6.2), Code tab, local session, Claude Code 2.1.285
• Claude iOS app, current App Store version
• Doc created by the local session through the Claude Docs connector; Remote Control is on for that session

Steps to reproduce

1. In a local Code-tab session in Claude Desktop, create a Claude Doc through the Docs connector.
2. Open the doc in the Claude iOS app, select text, add a comment that mentions @Claude.
3. Open the same doc in Claude Desktop and add a comment that mentions @Claude in a thread (I used "@Claude ping").

Expected

Comments from iOS and from Desktop are delivered the same way: the owning session gets the comment and replies in the thread. If delivery is impossible, the comment should be queued and delivered when the session is reachable.

Actual

• Desktop (step 3): the comment arrives in the session as a user turn [Artifact comment sent to Claude], and the session replies in the thread.
• iOS (step 2): the comment is saved on the doc (the Docs query API lists it with "to": ["claude"]), but the session gets nothing. The banner in the screenshot appears. The background comment watch doesn't pick it up either, and nothing is queued.

Observed timeline: a comment sent from iOS at 15:30 never arrived; comments in the same thread delivered from Desktop at 15:18 got a reply at 15:19.

Why it matters

For docs made in Claude Code, the only reliable path for comments is the desktop app on the machine where the session runs. The typical workflow — review the doc on a phone and leave comments — fails, and the user has to re-send every comment from the Mac or paste it into the session by hand.

Possible fix

Route Code-session doc comments through Remote Control (the session is already reachable from mobile that way), or queue them server-side and deliver on the session's next start, the same way the Desktop app injects them now.
