[Bug] Claude Code degraded response quality and instruction following regression

Bug Description
i don't know what you are doing on the backend here with the harness or the compute you're giving claude, but i have not changed my setup at all and claude has gotten exponentially more difficult to work with. i have to spend 10x the tokens for simple answers becuase i have to ask 6 times or peel back mutliple layers of claude not being clear or ansering other things rather than what i'm asking. i am getting real close to switching to openai or just going straight open source. this is no longer worth it. i get a lot of code out of claude, sure, but i have to then spend 20 times as long because claude made mistakes because it just didn't listen or outright ignored instructions or lied to me about what it did. so i end up saving zero time and probably waste more time using claude than i would have if i were coding of my own. the is a relatively new problem. there were always some issues, but for this last week, this is all i am seeing
