[Bug] Degraded response quality across Claude models: hallucinations and instruction non-compliance

Bug Description
Past 48 hours I have experienced many degraded experiences across Sonneet and Opus Models. Halluciations, not following instructions, a complete attitude change, dismissive and dodgign, and overall causing more issues than are getting fixed. Something has changed here and I dont know what!
