[Bug] Agent re-executes previously completed permitted file operation, wasting tokens

Bug Description
agnet was denied permission to change file, then the agnet was given the permission to do so, the agent as expeceted did the change, then two commmands laters the agent wasted tokens just to do the same loop that was alreayd permitted
