[BUG] Shows "monthly usage limit" message when the 5-hour session limit is reached

What's Wrong?

When I reach the 5-hour session limit, Claude Code shows a monthly limit
message instead of the 5-hour session limit message with its reset time.
This is misleading, because it implies I'm blocked until next month.

What Should Happen?

It should show the 5-hour limit message, e.g. "5-hour limit reached – resets <time>".

Steps to Reproduce

1. Use Claude Code until the 5-hour session limit is reached
2. Send any prompt
3. The monthly limit message appears instead of the 5-hour message
