[Bug] Claude frequently fails on simple confirmation and lookup tasks despite having necessary tools

Bug Description
i love that it admits that it got something wrong which is substantially better than me finding out constantly about it lying through omission or deliberatly. BUT it seems to get stuff wrong constantly when it was a s imple confirmation type of issue or lookup or simply using the fucking skills i gave it.
