[Bug] Claude frequently refuses valid tasks and ignores user instructions

Bug Description
Keeps refusing constantly to do what it's asked and does its own thing. I'm getting 50 to 100 refusals on tasks per day at the moment, and then I have to tell it, "No, this is what you need to do." Why is it refusing to do so much work? Why is Claude constantly delivering problems rather than solutions?
