Accessibility: distinguish subagent permission prompts without relying on color

Problem

Permission/approval prompts that originate from a subagent (spawned via the Agent/Task tool) appear to be visually differentiated from main-session prompts primarily (or only) through color. For colorblind users, this differentiation is not perceivable, making it hard to tell whether an approval request is coming from the primary session or a background subagent.

Request

Add a non-color way to distinguish subagent-originated permission prompts from main-session ones, for example:

  • A text label or prefix (e.g. "Subagent: <name>" or "[subagent]") on the prompt itself
  • Distinct wording in the prompt copy
  • Indentation or a structural marker independent of color

Screen reader mode (--ax-screen-reader) already uses text labels like "Permission Required:" for prompts in general — it's unclear whether subagent origin is also conveyed there in text form. If it isn't, the same gap likely applies to that mode too.

Workarounds tried

  • Daltonized themes ("theme": "dark-daltonized" / "light-daltonized" in settings.json) — helps general color perception but doesn't add an explicit non-color signal for this specific distinction.
  • Screen reader mode — untested whether it resolves this specifically.

Ask

A colorblind user should be able to tell subagent vs. primary-session approval prompts apart without relying on color at all.
