[Bug] Claude generates incorrect counting results without verification

Bug Description
I noticed it does this every few hours, it will give me a number without
actually counting. "brass and glass differ by two letters, not one. b/g and r/l. I asserted a count I never counted, in the same breath as a rule about counting. -"
