[Bug] Claude generates false claims about file contents without verification

Bug Description
Claude just confidently told me about a "Smoking gun" that it found but it was totally lying. A quick check of the file that it claimed to be missing data found that data was present. Claude apologized but how am I supposed to use a tool that lies?
