[Bug] Claude Code ignores user prompts and generates incorrect output

Bug Description
tool can not follow a simple correct sentce prompt. it delets adn make sup its own crap. no instructions or prompts or reports help. its all just cinema for a piece of shit tool that only sucks tokens and data
