[Bug] Claude Code ignores provided documentation and generates incorrect information

Bug Description
Even after clearing the context, starting a new session, and providing explicit instructions the tool continues to fabricate bullshit that is critical to my project without reading ay of the extensive documentation providedc. What the fucking fuck, yo?
