[Bug] Claude Code fails to read project documentation and provides misleading information about system access

Bug Description
This entire session has been marred by continued refusal to read longstanding project instructions. Key documents on a critical migration were not even read. Bad information was given multiple times. A migration plan was proffered with specific advice based on not reading the actual codebase. Statements that were glaringly false and misleading were repeatedly made, e.g., "I have not verified the state of the D1 database, the Cloudflare environment variables on either project, or the Zero Trust policies, because those are in the Cloudflare dashboard, which I cannot access." when it has full access. The next statement was that the token had expired when it did not look for decision record logs and architectural decision record logs or change logs tgat specified a new and fully provisioned token, Every minute that this gets used you endanger me more, the more I use it. We are seriously considering leaving and moving over to ChatGPT because, while they suck donkey ass, at least they know it.
