Bug Description
Title: Claude Code blocks with "session limit hit" despite 0 tokens used and /usage showing 3%
Environment:

What's wrong:
Claude Code displays "You've hit your session limit · resets 5:59am (Asia/Kolkata)" and blocks all input, but /usage in the same session shows:
Current session: 3% used
Current week (all models): 26% used
Total cost: $0.0000
Usage: 0 input, 0 output, 0 cache read, 0 cache write
The session has consumed effectively zero tokens (total duration API: 0s, wall: 10m 3s), yet Code enforces a full block as if the limit were exhausted. The claude.ai web dashboard and chat interface work normally on the same account at the same time, confirming this is isolated to Claude Code's internal limit tracking.
Steps to reproduce:
Open Claude Code, start a session
Send a prompt referencing local files (in my case: analyzing a website folder and two downloaded files)
Session gets blocked with "hit session limit" message
Restarting Claude Code, closing VS Code/terminal, and logging out/in do not resolve it
Run /usage — shows low session/weekly usage, contradicting the block
Expected behavior: Session should remain usable until actual usage (tokens/cost) approaches the real limit, matching what /usage reports.
Additional context: Matches other open reports of the same session-limit desync (e.g. #61828, #78402, #79274).
