[Bug] Claude frequently ignores instructions and hallucinates despite system prompts restart it and. synch

Bug Description
there alot of hhalicinations and my instructions are not followed it often makes it own mind up and does whateveer wants i have to keep a cllose eye on it too see if it has done what we agrd mor oftne than not it lies so i have to get it to what i originaly asked i. Opus 5 or Fabe does not fix these issues
