[Bug] Claude ignores memory file instructions and hallucinates code behavior

Bug Description
Claude frequently hallucinates about how existing code works makes confident claims about class behavior without reading the source, then defends those false claims when questioned. Also ignores rules stored in memory files (MEMORY.md) that were explicitly set to prevent this exact behavior. The memory system works for recall but Claude doesnt actually follow the instructions written there
This is a regression from the last 2-3 days. Previously Claude was much better at reading code before making claims and following memory file instructions. Now it confidently hallucinates about code behavior without checking, and ignores rules in MEMORY.md that it used to follow
