[BUG] Personal Claude Max account blocked — "organization has disabled Claude subscription access"

What's Wrong?

Claude Code CLI blocks all requests with:
"Your organization has disabled Claude subscription access for Claude Code · Use an Anthropic API key instead, or ask your admin to enable access"

This is a personal Claude Max account, not a Team/Enterprise org. /status shows:

• Login method: Claude Max account
• Organization: my address Organization

There is no admin who could have disabled anything, and no team/org admin to contact.

What Should Happen?

Claude Code should work normally with an active personal Claude Max subscription, as it did previously.

Error Messages/Logs

Your organization has disabled Claude subscription access for Claude Code · Use an Anthropic API key instead, or ask your admin to enable access

Also attempted to submit via /feedback in Claude Code itself, but that failed with:
Couldn't send feedback (server returned 403)

Steps to Reproduce

1. Log in to Claude Code with a personal Claude Max account (/login)
2. Run any command (e.g. a simple prompt)
3. Every request is blocked with the "organization has disabled Claude subscription access" error
4. Run /status — shows Login method: Claude Max account, Organization: (blank)
