[Bug] turning off plan mode doesn't actually turn off plan mode until the subsequent turn

Bug Description
If I shift+tab to cycle out of plan mode, I still get a plan response after sending the next message.

Something about the state management doesn't seem to be reflecting that I had already swapped out of that mode!

Ironically, this is particularly painful when I am using claude to do more extensive planning in text files. cc gets confused between the plan file I mean for us to collaborate on and its own plan. I don't want to 'plan to plan', I want to plan!
