[BUG] Multiple Claude Code versions appear in MacOS Privacy & Security settings

What's Wrong?

When I review System Settings > Privacy and Security > Files & Folders I see several Claude Code version numbers, not the application itself.

What Should Happen?

I should not see old versions of Claude Code appearing in the "Files and Folders" section of Privacy and Security on my Mac.

Steps to Reproduce

1. Install Claude Code
2. Wait for Claud Code to update
3. Approve required permissions
4. See that multiple instances appear
