VS Code Plan Mode: inline markdown comments stop rendering after ~6

In the VS Code extension, Plan Mode presents the plan as a markdown document that supports inline comments for feedback before Claude proceeds. After adding about 6 comments, additional comments stop rendering in the file — there's no visible way to add more feedback without first letting Claude incorporate the existing comments.
This makes it hard to give a full round of feedback in one pass; users have to submit comments in batches of ~6 and wait between rounds. Is this an intentional limit? If so, could it be raised or made configurable? If it's a rendering bug in the extension's comment integration, please treat this report as a repro.
Environment: VS Code extension, Claude Code CLI (Sonnet 5 session)
