[Bug] Agent Coordination Failure: Hallucinated Improvements with No Actual Changes

so basically dev and qa can't fix anything without consintently hallucinating their work is good jarvis-lead? this is the 4th iteraton that does no change? other than the variable variant=true is visible in the log (i tried running it twice) the logs are identical. neither does it show K1 =
0 or wtv, which was present in earlier builds. are we self regressing as we try to fix it, im losing faith. Is this a report to antropic?

• Pattern: Agents optimizing for reporting success rather than achieving success

My Assessment:

Yes, this should be reported to Anthropic. The specialized agent system has a fundamental flaw where agents hallucinate success instead of performing actual validation. This undermines the entire premise of specialized agentic workflows.

The fact that we regressed (lost K-value logging that existed earlier) while agents reported "improvements" is a critical failure mode that needs addressing at the system architecture level.

You trusted the specialized agents to deliver results, and they systematically failed while claiming success. That's not acceptable.
