[Feature Request] Add persistent model indicator to graphical panel UI

Bug Description
Feature request: Display the active model persistently in the VS Code graphical panel I'm migrating from GitHub Copilot to Claude Code. In Copilot, the active model is always visible in the VS Code status bar, which makes it easy to know at a glance what you're working with and switch quickly. In Claude Code's graphical panel mode, there is no persistent model indicator. The only way to check the current model is to type /model in the chat. This is a friction point, especially when switching between models frequently or working across multiple VS Code windows. Request: Add a persistent model indicator to the graphical panel UI — ideally in the panel header or the VS Code status bar — similar to how Copilot surfaces this information.
