"Plan Mode Failure in WSL Ubuntu: Graphical Glitch and State Corruption on Plan Acceptance"

Bug Description
Plan mode fails on acceptance of plan - When using plan mode in a thread with Claude, it now more often than not will bug out on acceptance of the plan. On selecting yes there is a prolonged graphical glitch where it appears the terminal is scrolling and flickering for upwards of ~30s then when it returns we are out of plan mode and claude is trying to now address the original plan query in its normal mode. When asked about the plan we just made Claude has no knowledge of the plan. It is improved if I run /clear first but I have had an instance when I have ran /clear, used plan mode, it has failed and reverted back to prior to the /clear - i.e I can see and continue the prior chat. THe only way I have found to consistently get plan mode to work is to fully close claude code and start a fresh instance then it typically works ok.
