[BUG] /effort command missing from VS Code extension slash menu (Pro plan, Opus 4.8)

What's Wrong?

Environment: VS Code extension version (check via Extensions panel → Claude Code → the version number shown there, or Command Palette → "Claude Code: " commands often show version in output)
Plan: Pro
Model active: Opus 4.8
What is tried: typing /effort in the prompt box, doesn't appear in the slash command menu at all
Comparison point: a colleague on the same extension version and same Pro plan has /effort working fine — this is the most useful detail since it rules out plan-gating or version-gating as the cause
Other commands work fine (/model, /usage) — so it's not a wholesale command failure
/status also doesn't work for you, if that's still true — worth mentioning as a possibly related data point
Steps already tried: window reload (no effect)
