[Bug] High rate limiting and API errors degrading service reliability

Bug Description
The amount of rate limiting and the amount of Opus 4.8 alert alerts as to high level of errors... it feels like the service is degrading. it's becoming difficult to have productive sessions using Anthropic. Getting concerned.
