[Bug] Sessions disappearing from resume list and Opus 4.8 rate limiting errors

Bug Description
I'm a Claude Max subscriber (3 months, no prior issues) using Claude Code v2.1.178, native install, on macOS / Apple Silicon. As of today, June 15 2026, I'm hitting two problems: Sessions disappearing intermittently. Several of my Claude Code conversations randomly vanish from the resume list — they are not archived and not active, they just stop appearing, and then sometimes reappear on their own later. I've confirmed the underlying session files are all intact on disk in ~/.claude/projects/ (100+ .jsonl files, ~5.8GB, some written within the last few minutes), so this appears to be a display/indexing problem, not actual data loss — but it's happening repeatedly and unpredictably. Messages not sending. When a session is available, I often can't send messages — I type, hit enter, and nothing happens. When it does error, I get: "API Error: Server is temporarily limiting requests (not your usage limit) · Rate limited." This is on Opus 4.8 while my usage is well within limits (5-hour ~51%, weekly ~59%, Sonnet ~4%), so it looks server-side, not a quota issue. I saw there was an "Elevated errors on Claude Opus 4.8" incident earlier today. Both started today and I've had zero issues like this in 3 months on Max. Is the session-disappearing issue a known indexing bug, and is the Opus throttling a known ongoing incident? My login, install, and files are all fine — the problems are the intermittent session visibility and the request throttling. Happy to provide a Feedback ID or debug logs. Thanks.
