[Bug] Command approval preferences not persisting across invocations in Fable 5

Question

**Bug Description**
When using Claude 4.8 Opus and presented with a command such as "git commit -q -m ' *" that I must approve, I would click Yes, and don't ask again, to which it would adhere and not ask again. With Fable 5 when presented with the same command and the same option from me, it seems to not store this information, and asks each time. This is significant with commands such as invoking the compiler on a test suite.
