[Bug] Command approval cache not persisting across sessions

Bug Description
Since some updates of Claude Code I am nearly constantly asked to allow commands I have allowed in the past. This feels like a changed behaviour and should be noted somewhere. It is annoying when I cannot left the agent alone for some time because it stops mid-work to ask for permission.
