[BUG] CC loses connection to Win 11 filesystem and hallucinates (?) their existence

Bug Description
Claude Code loses connection to the filesystem. Not working in a container. Writes files to a location which does not exist. When asked to use bash ls or dir, outputs files with checksums which do not exist on my drive.
