[Bug] Anthropic API Error: Rate Limited - Credit Consumed Without Review Generated

Bug Description
/ultrareview on branch feat/t068-ccip-migration failed with a server-side error — "API Error: Server is temporarily limiting requests (not your usage limit) · Rate limited" — and returned empty findings (no review produced). But it still consumed a free ultrareview credit: the counter went from "1 of 3" to "2 of 3". Requesting the credit be restored, since no review was delivered.
