[Bug] Usage credit consumed on failed ultrareview before completion

Bug Description
https://claude.ai/code/session_01BzJeKDbccyUtphwP2CDucJ is the third ultrareview that has failed and seemingly consumed a free credit. This has happened on two previous occasions. A usage crdit shouldn't be consumed until the process ends.
